Hawkeye Care Center provides Skilled Nursing, Long Term Care, Assisted Living and Assisted Living Memory Care in our beautiful west side location. We take tremendous pride in all we do and strive to provide the highest level of care possible to those we are blessed with serving. Our skilled therapy produces some of the area’s best outcomes, shortest lengths of stay and low return to acute through a collaborative holistic team approach to the care provided by our Physical, Occupational and Speech Therapists along with our nursing, dietary, activities, housekeeping, and social services staff. We work together with our residents and their families in developing a comprehensive plan of care and emphasize that each individual is unique, valued and cared for.
